Frequently Asked Questions About Your PPO Dental Plan
How do I find out if my dentist participates in United Concordia’s network?

You can search for a network dentist by visiting UnitedConcordia.com, clicking on Find a Dentist and selecting your network (listed on the enclosed benefit summary) and search preferences.

Are there advantages to visiting a network dentist?

Yes. Our network dentists agree to file claims for you, and to accept our allowances as payments in full for covered services. And, most of our dentists also accept our allowances for non-covered services or services above your annual maximum (these reduced rates are not insurance). That means you can save money by visiting a network dentist, because our allowances are typically lower than the dentist’s standard charges. And, United Concordia’s network dentists undergo rigorous review of their claims submissions and credentials, so you can rest assured you are receiving quality care.

If my dentist does not participate in United Concordia’s network, can I still see him/her?

Yes, you can receive care from any licensed dentist. If you do see a non-network dentist, however, your benefits may differ and your out-of-pocket expenses are likely to be higher than if you receive services from a network dentist. If you would like to nominate your dentist to be considered for participation in our network, you can submit his/her name by visiting the Members section of UnitedConcordia.com, selecting Forms and then clicking on Nominate Your Dentist.

What can my dentist bill me for?

What your dentist bills you for depends on whether you visit a network or non-network dentist. United Concordia network dentists can only charge you for applicable deductibles and coinsurance amounts. Non-network dentists can charge you for applicable deductibles and coinsurance amounts, and the difference between their standard charges and United Concordia’s allowances.

What information is available online?

In the Members section of UnitedConcordia.com, you can access forms, frequently asked questions and answers, a glossary of dental terms, and a dental health center of oral health articles, brochures, videos and kids’ pages. Once your plan is effective, your personal benefits information is available in our online member tool, My Dental Benefits. Once you register, you can review details on your coverage, eligibility, network, claim status and procedure history; print an ID card; and sign up for paperless EOBs.

What if I have other questions or concerns regarding the United Concordia dental plan?

Questions about your dental treatment should always be discussed with your dentist. For information about your benefits plan, visit us online at UnitedConcordia.com or call Customer Service at 1-800-332-0366.

Livongo is a new benefit for employees, spouses, and dependents who have Diabetes Type I or II and are covered under our Medical Plans.

Under the Livongo Program, enrolled participants will receive a connected meter, unlimited test strips, and coaching at no cost!

When you enroll in Livongo you will receive at no charge to you:

  • Connected Meter: Automatically uploads your blood glucose readings to your secure online account and provides real-time personalized tips.
  • Support from Coaches when you need it: Communicate with a coach anytime about diabetes questions on nutrition or lifestyle changes.
  • Unlimited testing strips at no cost to you: when you are about to run out, Livongo ships more supplies, right to your door.

How to enroll in Livongo:

  • Visit join.livongo.com/REGCODE/register to register and answer a few questions about you and your health. Next, download the Livongo app and log in.
  • Call Livongo Member Support at 1-800-945-4355
  • Registration code: GRAYANDSON

After enrolling, you will be shipped the Livongo Welcome Kit that includes the Livongo meter and all the strips and lancets you need to check your blood sugar. You will receive access to the Livongo member website, my.livongo.com, where you can personalize the program and access your readings.

You can cancel your membership at any time for any reason. Just call Livongo at 1-800-945-4355 or email help@livongo.com

Wellness

The Wellness Program Runs From April 1st to December 31st!

Please read below to learn how the 2023 Wellness Program will work! You can also earn a $50 or $75 gift card!

How do I earn a Wellness discount?

To earn your wellness discount for your 4/1/24 health benefits you must earn 300 wellness points by completing the following:

  • Complete an annual physical (between 1/1/23 - 12/31/23)
  • Complete your biometrics (between 1/1/23 - 12/31/23, submit physician form)
  • Complete any 2 preventative care visits or complete the health risk assessment.

After completing the requirements above, you will be wellness compliant!

EARN A $50 or $75 GIFT CARD
You can earn a gift card by completing even more points! You can earn additional wellness points by choosing from the additional activities on the points chart. To earn a $50 gift card, you will need to earn a total of 450 wellness points.

  • To earn a $75 gift card you will need to earn a total of 600 wellness points.

3 Ways Access the Wellness Portal

Online: There is a new Wellness portal. EVERYONE needs to complete the new wellness portal registration if you want to track wellness points online or on the app. Your username and password for the old wellness portal will not work.

  1. Go to allwell.medikeeper.com
  2. Click on the button that reads- Or click here to register.
  3. Type your first name, last name, date of birth, and zip code.
  4. Type your email address. This will be your username.
  5. Create a password.
  6. Then click submit.
  7. Click on yes, to agree to the wellness portal terms and conditions. And then click submit.
  8. Then click on the get started button.
  9. You now have access to the new wellness portal!

On the app

If you registered online:

  1. If using an Apple device, open the Apple App Store. If using an Android device, open Google Play.
  2. Search for “ALAVEDA” and then click “Get or “Install” to download the app. If you see a confirmation pop-up, click “Install” or “Confirm” to continue with the app download.
  3. Once the download is complete, click “Open” to open the app or navigate to your homepage and click on the Alaveda App icon.
  4. Click on Already Have a Login. Enter your existing username and password and click the orange “Sign In” button.
  5. You now have access to the wellness app!

If you have not registered online:

  1. If using an Apple device, open the Apple App Store. If using an Android device, open Google Play.
  2. Search for “ALAVEDA” and then click “Ge or “Install” to download the app. If you see a confirmation pop-up, click “Install” or “Confirm” to continue with the app download.
  3. Once the download is complete, click “Open” to open the app or navigate to your homepage and click on the Alaveda App icon.
  4. When you open the app, enter the registration code- GSMPI. Then click register.
  5. After you enter the registration code, complete the registration form and click on the submit button.
  6. You now have access to the wellness app!
